The Haven Transitions program offers informaton, resources and referrals to individuals and their caregivers who are coping with a life-limiting or terminal illness. Every month Haven Transitions helps more than 250 people who can benefit from pre-hospice care.

These services are offered at no charge:

• Supportive phone calls
• Volunteer assistance for companionship and respite care
• Children's anticipatory grief support group (call for locations)
• Information about and referrals to community resources
• Education to help with understanding care options and choices
• Advance care planning: living wills and advance directives
