Position Summary: Provides full range of hospice nursing care to patients and families in care center or home settings. LPN - PRN (As Needed) for Crisis Care

Position Summary:  Provides full range of hospice nursing care to patients and families in the care center under the supervision of a hospice RN as a supplement to the care provided by the RN. ARNP - FT

Position Summary:    The ARNP will manage selected medical problems, initiate, monitor or alter treatment plans and other activities for Hospice/Palliative care patients of HH network as defined within the framework of an approved protocol practicing under the direction of the HH medical director or HH associate medical director.  This position will serve in a consulting capacity to hospice clinical staff and others on issues of palliative management of symptoms and routine medical care.  Participates in medical coverage rotation, weekends and evenings.   

Education and Experience:  ARNP with current Florida licenses required 3-5 years experience as ARNP in palliative care related area preferred.  ANCC Board certification in palliative care nursing must be obtained up within one year of hire.  Hospice and/or long term care experience preferred.   Demonstrated ability to work non-judgmentally with persons of varied cultural, religious and socioeconomic backgrounds and lifestyles. Current Florida driver's license and insurance.


 

Customer Service Representative - PRN

Position Summary: Receives and responds to community, physician, patient and family requests for assistance. Records and processes referral and admission information and documents in patient information database.  Schedules registration/admission visits.  Prepares preadmission and admission paperwork and acts as a liaison between patients/families, referral sources, physicians and admission staff. Assists in distribution of admission information and reports to primary teams and physicians. Assists in processing daily reports reflecting census and referral admission status.

Education and Experience: High School or Equivalent, 1-2 years experience preferred in related healthcare position. Customer service experience required. Word processing/data entry skills. Position requires excellent listening, communication and organizational skills, accurate entry of documentation into patient information database, and ability to handle multiple tasks/calls.  Valid Florida drivers license and insurance required.


RN Admissions FT

Position Summary: Visits patients and families for the purpose of providing information and admitting to HH.   Develops Plan of Care for HH services, obtains physician certification, primary and secondary diagnosis and admission orders for patients who qualify. Acts as liaison in coordination of referral and admission information with referring and attending physicians and their staff and HH primary teams. Acquires, documents and distributes patient, physician and vendor information for primary teams and affiliates in a timely manner.

Education and Experience:   Current Florida RN license with 2-3 years nursing experience, preferable in oncology, palliative care, geriatrics or home health care. Minimum 1 year customer service/sales experience required.  Excellent interpersonal and customer relations skills required. Experience working with interdisciplinary team desired.   Computer skills including Word, Excel, and internet use preferred. Valid Florida Driver's License.


Hospice Care Aid (CNA) - FT and PRN (As Needed)

Position Summary:   Provides personal care services to patients in care centers.  Provides light housekeeping and meal preparation as needed.  Due to business needs, may be required to work different shifts, teams and location.

Medicaid Coordinator – FT

 

Position Summary: This position is responsible for the Medicaid application process by assisting the families and teams in the application process.  Works with all responsible DCF offices and maintains a strong professional relationship.

 

Education and Experience: AA Degree required, Bachelor of Social Work preferred.  Working knowledge of Medicaid application process, with 2 - 3 years of experience preferred.  Valid Florida Drivers license and Insurance required.

 


Facilities Assistant - FT

Position Summary: Maintains building, cleans and keeps in orderly condition all areas of facility as assigned.  Performs routine inspections, care and cleaning of furniture and fixtures (i.e., windows, spot clean carpet and chairs).  Assist all departments (including satellites when assigned) with tasks requiring basic mechanical knowledge, use of tools, or moderately heavy lifting and/or labor. Performs routing inspection of HVAC, refrigeration and other building machinery as assigned. Assists in preventative maintenance programs as assigned. Assist with preparing facility for special functions.  May require some weekend hours.   

Education and Experience:  High School diploma or equivalent, 1-2 years experience in cleaning and basic facility maintenance. HVAC certification preferred.  Basic knowledge of carpentry, plumbing, electric, air conditioning and mechanical/electrical equipment, painting and drywall repair, pressure washer and cleaning equipment.  Awareness of safety/OSHA regulations.  Florida driver’s license and auto insurance.                                                      


Receptionist - PRN (As Needed)

Position Summary:  Provides clerical and phone support to Primary On-Call team. Answers and directs incoming calls for the agency/network. Performs other essential office / receptionist duties as needed, including but not limited to copying, faxing, filing, processing physician orders, data entry.

Education and Experience:  High School or Equivalent, 1-2 years related office experience preferred. Hospice experience preferred. Experience and skill preferred in multi-line telephone use and dissemination of patient information. Excellent written and verbal communication, organizational and interpersonal skills required. Director of Corporate Compliance - FT

Position Summary: Responsible for development and implementation of a comprehensive Corporate Compliance Program.  Manages Compliance Department.  Conducts Compliance investigations and coordinates with the SantaFe HealthCare Compliance Department. Assists in the investigation of reports made to the SantaFe HealthCare compliance hotline.  Serves as Privacy Officer and monitors external and internal environment for compliance issues.  Plans and directs organizational actions to ensure compliance with state, federal and accrediting guidelines for operations.  Serves as a research resource to staff regarding laws, policies, benefits, and administrative rules, including but not limited to the OCR regulations pertaining to HIPAA Privacy, the CMS regulations pertaining to HIPAA Transactions and Code Sets, National Provider Identifier; and Security standards; and Medicare and Medicaid CoPs.

Education and Experience:   Bachelor's degree required, Masters or Law degree preferred; previous general management experience preferred.  Minimum five years experience in the healthcare industry required; Hospice experience preferred.  Thorough knowledge of state, federal health services and Hospice care regulatory rules and guidelines preferred.  Previous experience in planning, audit, budgeting, procedure and policy implementation, reporting, operational review and analysis, and project management are necessary.  Demonstrated performance in: strong leadership skills, ability to get things done through others, using informal authority within a matrix organization; analytical, communication, presentation skills; proficiency in Internet use, word processing, spreadsheet, database, and presentation software applications; excellent organization abilities, effective use of company resources; ability to manage multiple priorities and concentrate on what is significant.

 


Director of Training & Development

Position Summary: Responsible for the development, administration and implementation of the overall strategy of training and development for Haven Hospice. Includes clinical and on boarding training.  Ensures the development of educational programs that are in compliance with state, federal, local laws, rules and regulations and are congruent with organizational and corporate goals.  Facilitates and coordinates a collaborative interdisciplinary work group to develop new effective educations programs and services.  Plans and organizes the training and development operations linking internally with business units and externally with customers. Develops relationships with colleges, universities and other outside training providers. Maintains the provider ship for licensed CE programs.

Education and Experience: Bachelors degree required, Master's degree in Public Health, Health Education, or Clinical Nursing preferred.  Licensed in the State if appropriate.  Minimum of 5-7 years of experience in corporate education setting.  Minimum of 5 years progressive supervisory/management experience required.  Strong interpersonal skills, leadership abilities, analytical problem solving, independent decision skills essential.  Ability to work flexible hours with frequent disruption of routine to meet network needs.  Excellent verbal and written communications skills.  Knowledge of adult education techniques.

 


Manager of Retail Operations - FT

Position Summary:  Responsible for the overall vision, direction, coordination of the Haven Hospice Attic Resale stores.  Oversees and is accountable for business practices to ensure maximum sales and revenue through: volunteer recruitment and retention, donation intake and processing, inventory tracking, competitive pricing, expense management, human resource management, annual & quarterly sales plans, attaining weekly and monthly sales goals and attaining desired annual Net Profit Margin. Is responsible for planning, executing and tracking donation drives, the Merchant Donor program and the community special needs program. Is responsible for compliance to set policies and procedures specific to cash processing and donation intake. Ensures customer and donor service satisfaction.  Is responsible and accountable for department budget preparation

Education and Experience:

AA degree required. BA degree preferred.  Must demonstrate 5 years of progressive management experience preferably in a retail setting.  Including managing and developing staff/volunteers, addressing personnel performance issues, completing human resources functions such as recruitment, interviews and corrective action;  operating a store within budgeted expense levels, forecasting, strategic planning, establishing revenue, sales and goals, making process improvements that improved margins and completing weekly/monthly/annual reporting (sales, inventory, budget stats, etc.). Customer service experience required.  Excellent inter-personal, communication and organizational skills.  Ability to develop a community outreach program and partner with the business community.  Ability to work with all levels of Hospice personnel, Hospice volunteers and the general public.  Excellent computer skills required.  Driver's license and insurance.

Position Summary: Provides administrative and project management support for Executive Director and other directors as assigned. Ensures smooth operation of executive office.  Valid Florida Drivers license and Insurance Required.

Education and Experience: Post high school educational attainment required, Associates Degree or higher preferred with 7-10 years progressive administrative/executive assistant experience preferred. Excellent verbal, written and communication skills required. Advanced PC /database/word processing/spreadsheet /power point skills required.


Associate Director of Development

Position Summary:  The Associate Director of Development/Organizational Giving will serve on the management team.  Responsible and accountable for targeting, cultivating organizational and business partnerships throughout the network that enhance fundraising activities.  Responsible and accountable for planning and implementing all aspects of donor cultivation, management, stewardship for assigned territory.  Responsible and accountable for all aspects of capital and planned gifts for this service area.  Responsible and accountable for creating and maintaining donor profiles in the Raiser's edge software.  Coordinates necessary volunteer and staff participation in appropriate events and activities.  Actively participates in departmental strategic planning/implementation and will have key role in meeting annual development revenue goals.

Education and Experience:    Bachelor's Degree required, Master’s degree preferred.  2-5 years of demonstrated successful fundraising history essential. Knowledge of annual giving, capital campaigns, donor stewardship and public relations desired.  Problem solving and organizational skills, and excellent written and oral communication skills essential.  Excellent analytic and strategic planning abilities, strong interpersonal skills required and have demonstrated ability to work well with diverse constituency.    Must have proficient computer skills with Microsoft Office Suite.  Must have valid Florida Driver's License and insurance.

Position Summary:  This position is 75% patient care with a 3/4 caseload and 25% administrative support to team counselors and PCC/team manager/satellite administration. Provides full range of hospice SW services and bereavement support to patients & families in homes and institutional settings.  Works with interdisciplinary team to provide directly, or through coordination of team members, appropriate assessment of and support to staff, patients and families.  Serves as liaison with other community resources; assists staff, patient’s physician and volunteers in understanding significant social and emotional factors affecting a patient and family during life limiting illness. Provides assessment for normal and complicated grief and provides grief bereavement counseling as appropriate. Mentors team social workers, coordinates caseload with PCC, reviews documentation and conducts related meetings. Assists team manager and PCC on personnel issues, including interviewing and hiring. Coordinates with PCC in covering PCC's responsibilities during absences.  May assist with grants. Works assigned on call shifts.

Education and Experience:  Master's in Social Work. Current license as a Clinical Social Worker required. 1-2 years experience in field. Hospice experience preferred. Position Summary: Responsible for food preparation, cooking, clean up and maintenance of kitchen and dining areas. Assists kitchen coordinator with all tasks required to provide quality meals meeting patient's dietary requirements and maintain clean kitchen and dining areas. Follows policies and procedures in regard to Agency, OSHA and Health Department regulations. Works with and without direct supervision of Dining Services Coordinator. May require work in early mornings, late evenings and weekends as necessary.

Education and Experience: High School Diploma or equivalent with one year of commercial food preparation experience. Food handling certification required. Knowledge of Health Department and OSHA regulations related to kitchen operation and food handling preferred. Ability to work alone or with a team. Repair/Delivery Tech DME - PRN

Position Summary: Responsible for the delivery, set-up, maintenance and repair of durable medical equipment uses in the home. Instructs patients and/or caregivers on use, care and basic maintenance of the equipment when appropriate. Assists in warehouse with cleaning equipment, equipment repairs, medical gas distribution and maintaining inventory. Completes accurate driver logs, delivery sheets and other paperwork. Responsible for properly maintaining assigned delivery vehicle. Works assigned on-call shifts.

Education and Experience:  High School diploma or Equivalent. Must maintain a current Florida Driver's License and a safe driving record. Knowledge of Home Medical Equipment is preferred. Must have basic mechanical skills.

 


Housekeeper  - PRN

Position Summary:   Cleans and keeps in orderly and homelike condition all areas of the Hospice Care Center, including patient rooms and bathrooms, hallways, living rooms, dining room, staff lounge and bathrooms.

Licensed Massage Therapist - PT

Position Summary:  The Licensed Massage Therapist (LMT) will be responsible for identifying patients appropriate for massage therapy in conjunction with interdisciplinary team (IDT) referrals, developing a massage therapy plan of care for each patient receiving a massage, evaluating and determining massage precautions and special considerations that require modifying/adapting a patient’s massage therapy plan of care for each patient receiving massage, and providing massage therapy interventions appropriate for hospice and palliative care to patients referred within the LMT’s team assignment. Completes documentation according to agency policies and procedures. Communicates with members of the IDT regarding the patient’s current status.  Educates IDT members about massage referrals most appropriate to the LMT’s skills base. 

 Education and Experience:  Current Florida Massage Therapy license (LMT) required. Must meet one of the following four criteria. (1) Minimum of one-year full time professional experience as a LMT, preferably in a hospice or palliative care setting. (2) Minimum of two years full time professional experience in an allied health profession (e.g., RN, LPN, Reiki practitioner or Psychotherapist where patient sensitivity and/or palpation skills are emphasized). (3) Appropriate experience in caring for the terminally ill, geriatric clients/patients, people with disabilities, or similar experience. Position Summary:  Ensures spiritual care services and bereavement support to patients and families in homes and institutional settings. Coordinates with interdisciplinary team and community clergy to provide directly or through coordination of appropriate spiritual assessment and support to patients and families.  Coordinates with local clergy as patients need. Conducts religious services and rituals as appropriate, to provide support to patients and families. Provides spiritual counseling for patients and families as needed to the extent not provided by the patient and families existing clergy. Participates in local clergy associations/groups.  Develop and coordinate a network of clergy volunteers.  Meets productivity expectations as required.

Education and Experience:    Master of Divinity degree or equivalent from an accredited school/seminary. Four units of Clinical Pastoral Education preferred. Minimum of one-year pastoral experience as a Chaplain. Demonstrated ability to work non-judgmentally with persons of varied cultural, religious and socio-economic backgrounds and lifestyles.  Understanding of death and dying issues and an ability to provide pastoral care to the dying. Coordinates, implements and proactively maintains a system of positive contact and outreach with appropriate HNCF staff who will collaborate on account development plans . Position Summary:  Responsible for HIM functions and analysis required to maintain accurate and organized patient records. Volunteer Specialist - Gainesville - PT

Position Summary: Coordinates all volunteer activity including recruitment, training, placement, scheduling and retention of volunteers for Haven Hospice Attic Resale Stores.  Oversees support meetings, recruitment events, training and recognition events for volunteers serving in the Attics. Promotes the Attic to teens through the retail teen program, and oversees the teen participants.   Actively participates in team meetings with volunteer staff and Attic staff.

Promotes Haven Hospice and the Attic Resale Stores within the community through networking and presentations to churches, civic organizations and other opportunities; schedules volunteers to help promote Haven Hospice at events and festivals.
