Long-Term Care

We know how important it is for you to be fiscally responsible. Our service promise includes:
  • Room and board payment: We put your check in the mail within 17 days of receiving your invoice.
  • Medicaid pending: We accept Medicaid-pending patients who are eligible for and clinically appropriate for our care, and we assume financial responsibility for their room and board – meaning we accept responsibility for paying you.
  • Medicaid assistance: We have an expert Medicaid liaison for you to work with, regarding any billing issues for Medicaid residents.
We know how important it is for you to keep your residents in your facility. Our service promise includes:
  • Inpatient hospice care: If you meet the regulatory requirements for staffing, we can collaborate to provide inpatient hospice care in your facility – increasing your revenue, keeping residents in the facility and optimizing care.
  • Continuous care: For residents in crisis, we offer continuous care to enable the resident to stay in the facility, even with an exacerbation in symptoms. Continuous care, for residents whose clinical condition makes them eligible, enables us to provide greatly enhanced clinical staffing interventions from our nurses, home health aides and other staff, compared to those which we provide an otherwise stable resident.
  • After-hours service: When you call us, any time of day or night, you reach a Haven Hospice staff member who has immediate access to information about the resident about whom you are calling. In addition to being able to assist you on the phone, we can call upon our staff to visit the resident at the facility whenever necessary. For patients whose unique needs require regular weekend visits, we are able to work with you on a patient-bypatient basis to meet their/your needs.
We know how important it is for you to achieve excellent survey results, and we understand the challenges end-of-life care residents create at survey time. Our service promise includes:
  • Care-plan coordination: We can be present at each resident care conference to assure care-plan coordination and accurate documentation. Our collaboration includes a customized collaborative service plan that shows surveyors a signed record of our collaboration to determine who is going to do what, when.
  • Survey support staff: At your request, we provide a Haven Hospice staff member with significant survey expertise to assist with any survey issues that arise without shared patients. You can reach our survey support staff by phone, and they are available to come to your facility anytime.
Responsible
  • Continuing-education programming: Haven Hospice has a broad array of CEU-approved in-service programs that can assist you in ensuring that your staff is appropriately maintaining their licensure to meet pertinent regulatory requirements.
  • A long track record of successful collaboration: We have worked with more than 75 facilities and collaborated to serve over 1,600 patients in north Florida. Each year we experience over 35 surveys; we know what is necessary to collaborate with facilities to make sure that our shared patients do not present survey problems.
We know how important it is for you to deal with the staffing challenges that all facilities face.

Our service promise includes:
  • Psycho-social support: Haven Hospice social workers supplement your team by providing direct patient support and being available to interact with family members and loved ones by phone, at your facility or at the loved ones’ home. In addition, we recognize that your staff experiences loss when residents die; our staff is also available for their support.
  • Spiritual support: Haven Hospice chaplains, of many denominations and cultures, are available to provide spiritual support to residents and their loved ones. As with Haven Hospice’s social workers, our chaplains are also available to support your staff. Together, the Haven Hospice spiritual/psycho-social team can also provide memorial services in the facility for staff and for other residents who often experience loss when their friends die.
  • Flexible staffing to meet the plan of care: When a patient’s plan of care requires weekend or evening staffing, we work with you on a patient-bypatient basis to meet the needs of the resident.
  • Medical support: At the request of your medical director and director of nursing, Haven Hospice physicians are available for palliative-care consultation or to act as the resident’s attending physician, if that is appropriate.

Advanced Care
  • Customized “We do it your way” facility profile: Our staff meets with your staff to determine how we can best be your supplemental care partner, interacting in accordance with the operational practices of your facility. For example, we check with you regarding:
    • How we shall check in and out of the facility
    • Where we should park
    • Whether we shall supplement our name badges with identification from your facility
    • How we shall work with your pharmacy vendor
    • How we shall handle referral requests – with whom we collaborate to assure a program that works for your needs
    • How you wish us to deliver and pick up equipment needed by our joint patients
    • How we can accommodate differences if your after-hours procedures are different than regular-hours procedures
    • How we can work together to educate prospective patients and families and appropriately interact with patients and families likely to need, but not yet ready for, our services We distribute this information to our staff, so that they know all of your requirements. Of course, we regularly ask you for the formal opportunity to review this list and update it, based upon your understandably changing needs.
  • Administrator on call: We know that there are circumstances in which you need to reach an administrator in our organization. Twenty-four hours per day, seven days per week, you can call us and reach an administrator who has the training, competence and authority to satisfy your needs.
