What is Hospice Care?
Hospice care provides palliative care to individuals diagnosed with a life-limiting illness as well as supportive services to their loved ones. It is a continuum of care ranging from routine home care to inpatient care with emphasis on maintaining the patient at home in their most familiar, comfortable and least restrictive environment. At Haven, we offer comprehensive care and support to our patients and their families each and every day. We’ll help your loved one to be as comfortable as possible and give your family and caregiver the resources and assistance they need to help care for them. Our mission is to provide comprehensive and compassionate care while respecting needs, beliefs and wishes.
Services Provided by Haven
Routine home care — provided in the loved ones home or wherever they reside, be it a nursing home or assisted living center.
Continuous home care — provided during times of crisis to help keep your loved one at home while our patient care team helps to relieve acute medical symptoms and bring them under control.
Inpatient respite care — provided in an approved Medicare facility, such as a freestanding hospice facility, hospital, nursing home or other long-term care facility on a short-term basis to provide a break for family members or other persons caring for a loved one at home.
General inpatient care — provided in an approved facility when specialized care is needed to relieve pain and bring chronic symptoms under control. These facilities may include a hospital, a hospice with inpatient units, or a skilled nursing center.
Support Provided by Haven
These are services provided by a Haven physician who specializes in end-of-life care or by a personal physician; home visits supervised by a registered nurse; and services provided by a certified home health aide. Haven provides hospice care wherever the patient calls home. Most often care is provided in the patient’s home but is also available in hospitals, nursing homes, assisted-living facilities and at dedicated inpatient care centers.
Our interdisciplinary team of doctors, nurses, certified nursing assistants, nutritionists, social workers, chaplains and volunteers do everything possible to meet the physical, emotional, social and spiritual needs of every patient, family member and caregiver.
At Haven Hospice, we do this by actively and aggressively managing pain and other distressing physical symptoms while providing emotional and spiritual care so that you and your loved ones can focus on living. We employ a variety of tools to measure quality and use the data we gather to improve patient care. One set of data which is particularly important involves patient pain scores. Haven conducts internal, quality care surveys and hires an outside consultant to conduct external audits to ensure the highest quality of care and services.
The Florida Department of Elder Affairs standard for decreasing pain to a level of five or less by the end of the fourth day is 50 percent. At Haven Hospice, our standards are much higher. Internal reports indicate that we have achieved over 95 percent for 2012, 2013 and 2014.
Hospice patients who are enrolled in Medicare will find that their hospice care is fully covered by the Medicare Hospice Benefit. The Florida Medicaid Hospice Benefit and most private insurance plans also cover hospice care. As a not-for-profit, community hospice organization, we do not turn anyone away. Hospice services are provided regardless of an individual’s ability to pay. The generous contributions we receive from the community enable us to care for anyone in need.
