Haven Hospice is your not-for-profit, community hospice organization providing services since 1979 and licensed in Florida since 1980. Haven is North Florida's expert in end-of-life and palliative care, receiving national recognition as a Circle of Life Award Recipient from the American Hospital Association for its excellence and innovation. Haven has also been recognized as a Florida Pacesetter for its leadership in promoting advance directives. For more than 35 years Haven has had the honor and privilege to serve more than 70,000 patients and families in North Florida.
Haven programs include home and inpatient hospice services, palliative care consultation services, transitions support, community education and resources, as well as grief support.
Patients, friends and family can initiate hospice services. The referral to Haven Hospice is as simple as a phone call to 1.800.HOSPICE or you can complete an online referral form.
Haven Hospice takes care of you wherever you call home: at your residence, assisted living facility, nursing home or in the hospital.
Specially-trained Haven nurses who have access to your patient’s medical records are available by phone 365 days a year to answer patient and caregiver questions. Nurses are also available for home visits during off-hours.
Haven provides the medications, medical supplies and medical equipment that relates to your hospice diagnosis. Because we own a medical equipment supply company, we can ensure the quality and type of equipment that you receive.
Haven Hospice also has five free-standing, state-of-the-art care centers in North Florida providing a comfortable, home-like setting where family members are welcome 24 hours a day and are encouraged to stay.
Haven Hospice encourages family pet visits at our care centers, offers volunteer pet visits wherever you may call home and provides the Pet Peace of Mind ® program to help patients complete their end-of-life journey without worrying about their pet’s current or future needs.
Grief support services are available to all of Haven’s patients and families. With specially-trained clinical staff, we are also able to provide the kind of care that best meets the unique physical, spiritual and emotional needs of veterans.
What happens when a person has a serious or life-limiting illness but is not yet ready for hospice care? Haven Hospice offers emotional support, education, information and referral as well as limited volunteer assistance through the Haven Transitions program.
For non-hospice patients with a life-limiting illness, Haven offers palliative care to provide relief from the effects of illness, regardless of the diagnosis or prognosis. The overall goal is to improve quality of life for the patient and family including the body, mind and spirit.
